Job Description POSITION DESCRIPTION POSITION TITLE: ASSISTANT DIRECTOR OF NURSING DEPARTMENT: NURSING RESPONSIBLE TO: DIRECTOR OF NURSING Assists the Director of Nursing Services in directing the activities of licensed and non-licensed personnel who provide health care and nursing services to Residents of the Health Care Center. primarily. Serves as wound nurse, staff development nurse, infection control specialist and employee health nurse. Serves as interim DON in absence of the Director of Nursing, and resource/support for Independent and Assisted Living. RESPONSIBILITIES: Coordinate, monitor, and evaluate facility’s wound care program. Track and trend all new and existing ulcers; work with physicians, Vohra Wound, and unit coordinators to identify prevention strategies for high risk Residents. Work and round with Wound Physician weekly to ensure affected Residents are assessed, treated and that staff adhere to treatment plan. Work in tandem with Clinical Specialist to prepare and evaluate QAPI required documentation. Track and trend wounds, offer treatment recommendation to staff and unit coordinators as needed. Share leadership of QAPI meetings with Clinical Specialist. Apprise DON of all acquired wounds on an immediate basis. Coordinate, monitor and evaluate facility’s infection control program; track and trend infections and report to DON on a monthly and quarterly basis. Perform surveillance to ensure standards of Infection control are met. Keep facility staff abreast of changes in Infection control practices, update policy and ensure department specific training covers infection control issues; in cases of communicable disease outbreaks, work with the Administrator, D.O.N. and public health department to ensure accurate reporting and compliance with regulations to prevent spread of communicable diseases. Keep Director of Nursing informed of Resident care and nursing department staffing concerns. Rotate on-call responsibility per DON’s assignment. Monitor unusual occurrences and employee illness or injury; consult with managers and supervisors on performances issues; and educate staff as needed regarding medication errors and other areas of care deficits. Ensure each employee’s health file is complete and current for PPD and/ or Chest-X-ray as well as Hepatitis vaccine program. Support standard precautions and blood borne pathogen training, follow-up with any blood borne exposures and report according to OSHA standards; coordinate training of hazardous materials & safety procedures Insure precautions are taken to keep the facility in compliance with nursing home regulations and nursing practice standards. Resource for staff regarding state regulations, facility and Department policy and procedure, and practice standards Assist DON as needed with the interviewing staff. Assist in evaluating, coaching and disciplining staff. Counsel/Coach staff as needed according to guidelines of the Human Resources department and the Director of Nursing. Assist in reviewing charts of potential Residents. Become certified to complete the UAI, in order to assess IL and AL Residents. Work with nursing team to review and revise policies; keep policy & procedure manuals for Nursing updated. Coordinate/Conduct orientation for all newly hired nursing employees. Notify staff and D.O.N. if extra orientation is needed. Develop and measure nursing staff competencies with educational needs assessments; oversee CPR training of nursing/rehab/AL staff; Heimlich training for nurse aides. Complete educational needs assessment; ensure staff completion of all mandatory data base training with the goal of 100% compliance each month. Develop, present, document, evaluate education programs to meet nursing needs related to: new technology, products, new patient population, procedures, regulatory mandates, quality deficits or improvements. Consult on patient care, patient assessment or patient teaching Suggest monthly education for all staff; Keep current with Relias updates. Coordinate training on relevant topics to ensure facility compliance with state and federal regulations related to same. Must maintain privacy and confidentiality of Resident information at all times; round on units and make recommendations regarding nursing operational needs In the absence of the Director of Nursing, perform the duties of the Director Any other duties assigned by Director of Nursing QUALIFICATIONS: A registered nurse licensed in the Commonwealth of Virginia. Able to assess changes in Resident’s conditions CPR certified Able to function in any of the positions for which she/he is training staff Able to recognize and solve problems, make decisions, cooperate with people and supervise staff Display enthusiasm, leadership ability and tact Able to take weekend on-call coverage for the facility as assigned Fill in as supervisor, charge nurse. etc., at DON’s request Willing to teach, coach, encourage and counsel staff (while communicating all to Director) Certification in Gerontological Nursing preferred Able to set priorities and working a fast paced environment amid completing needs Knowledge of nursing home regulations, nursing practice Fiscal responsibility Able to work with frequents interruptions Willing to work beyond normal working hours, on weekends and in other positions temporarily, when necessary Able to work a varied schedule providing training to each shift as needed Will attend and participate in continuing educational programs designed to keep employees abreast of changes in their professions Will attend mandatory in-services conducted with the facility Able to move intermittently throughout the workday Able to cope with the mental and emotional stress of the position Possess sight/hearing, or use prosthetics that will enable these senses to function adequately so that the Requirements of this position can be fully met, to include use of departmental equipment and/ or supplies Function independently, have flexibility, personal integrity, and the ability to work effectively with the Residents, personnel and support agencies Able to relate to and work with ill, disabled, elderly emotionally upset and at times hostile people within the facility May be required to lift, push, pull and move equipment, supplies, etc., in excess of 50 lbs. throughout the work day, getting assistance and using appropriate equipment such as gait belt is a must! Able to read, write and speak English in an understandable manner Possess the ability to make independent decisions when circumstances warrant such action Possess the ability to deal tactfully with personnel, Residents, family members, visitors, government agencies/ Personnel and the general public Able to follow oral and written instructions Have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle Residents based whatever maturity level that are currently functioning Be knowledgeable with Care-tracker and able to train others on use ACKNOWLEDGEMENT: Every effort has been made to keep our position description as complete as possible.
